第三节 巨幼细胞贫血
巨幼细胞贫血(megaloblastic anemia,MA)主要是由于叶酸和(或)维生素B 12缺乏,使DNA合成障碍,但胞质中RNA、HBG的合成不受影响,最终导致巨幼红细胞形成,表现为“核幼质老”发育不平衡的改变。粒系和巨核系细胞也可以发生巨幼变,最终导致细胞寿命缩短,发生无效造血。外周血常表现为全血细胞减少和大细胞性贫血。
1.血象
单纯巨幼细胞贫血表现为大细胞正色素性贫血,RBC计数和HBG浓度的下降不平行,RBC数下降更明显。外周血RBC大小不等,以椭圆形大红细胞多见,形态不规则,着色较深,也可见巨红细胞、点彩红细胞、含H-J小体或巨幼变的有核红细胞(图2-39,图2-40)。如伴有缺铁性贫血,可出现双形性贫血特征(图2-41)。网织红细胞正常或轻度升高。WBC数量正常或减低,可见巨幼变的中性多分叶核粒细胞(5叶以上)(图2-42,图2-43)。血小板计数正常或减低,可见巨大血小板(图2-44)。
2.骨髓象
骨髓增生活跃或明显活跃(图2-44,图2-45),粒红比值降低或倒置。红细胞系统增生明显伴巨幼变,巨幼红细胞常大于10%。巨原红、巨早幼红细胞比例增高(图2-46~图2-49),有时可见核畸形、核碎裂和多核巨幼红细胞。易见嗜碱性点彩红细胞或幼红细胞、分裂细胞及H-J小体(图2-50~图2-54)。粒系细胞相对减少,各阶段均可发生巨幼变,以巨晚幼粒和巨杆状核粒细胞居多,也可见巨多分叶核中性粒细胞(图2-55~图2-56)。巨核系细胞也可巨幼变,可见核分叶过多和核碎裂的巨核细胞(图2-57~图2-60)。淋巴细胞形态常无明显改变,偶见单核细胞巨幼变。
